Rapid Rehousing & Homeless to Home Program
At Freedom Project, our Rapid Rehousing program for formerly incarcerated individuals provides safe, stable housing as a foundation for healing, restoration, and self-determination. Through our Homeless to Home model, we meet people where they are, walking alongside them with trust, dignity, and care.

What we do
Our program is designed to support single adults and families experiencing homelessness, housing instability, or transitioning from systems of harm such as incarceration or domestic violence. We help participants secure permanent housing while creating customized support plans rooted in their lived experiences, strengths, and future goals.
Our approach
- Healing-centered
- Peer-informed
- Trauma-responsive
- Grounded in liberation and community care
How the Rapid Rehousing Program works
Individualized Housing Navigation
We partner with landlords, housing authorities, and agencies to reduce barriers and help secure affordable, safe homes.
Functional Housing Support Plans
Participants co-create a plan outlining goals, resources, and check-ins with a housing support coordinator.
Time-Limited Rental Assistance & Flexible Funding
We offer flexible financial support to help cover deposits, arrears, move-in costs, or short-term rent.
✅ Wraparound Case Management
Our team connects participants with employment, behavioral health, peer mentors, reentry support, and community wellness services.
✅ Healing-Centered, Peer-Informed Approach
Led by staff and volunteers with lived experience, our program ensures cultural humility, restorative practices, and accountability.
